3. Under Patient Name or Account No., indicate the PATIENT’S FULL NAME or PATIENT ACCOUNT NUMBER.
4. Fill out your payment information to complete the transaction.

You will be able to complete your payment and receive a receipt via email. IMPORTANT: BE SURE TO INDICATE PATIENT’S FULL NAME OR ACCOUNT NUMBER UNDER “PATIENT NAME OR ACCT NO”. Thank you!
